How GLP-1 Weight Loss Can Accelerate Aging —and What You Can Do About It

If you’ve lost weight on a GLP-1 medication like semaglutide or tirzepatide, you may have noticed something unexpected staring back at you in the mirror: hollow cheeks, more visible lines, and skin that doesn’t sit quite the way it used to. This phenomenon — often called “GLP-1 face” — has become one of the most talked-about side effects of rapid weight loss, and it’s a topic our providers at DermaTouch RN are asked about almost daily.

The good news is that facial volume loss and skin laxity after GLP-1 weight loss aren’t something you simply have to live with. With the right combination of skin-tightening treatments, biostimulators, and a proactive skincare routine, it’s possible to support your skin through the process, or even get ahead of it before changes set in. What Is “GLP-1 Face”?

“GLP-1 face” is a term that’s emerged to describe the facial changes some people notice after significant GLP-1 weight loss with medications like semaglutide or tirzepatide. It’s not a clinical diagnosis, but it does describe a real and increasingly common phenomenon our providers are seeing in practice.

As the face loses fat quickly, the results can show up as hollowing in the cheeks, temples, and under-eye area. Skin may also appear looser or less supported than before. The face may look more tired, gaunt, or aged, even though the weight loss itself is a positive outcome.

It’s important to note that GLP-1 face isn’t a flaw in the medication or in how someone’s body responds to it. It’s simply a side effect of losing fat from an area where it once provided structural support. Understanding why this happens is the first step in knowing how to address it, which we’ll cover next.

Why Does GLP-1 Weight Loss Change the Face and Skin?

Darbonne points out that “GLP-1–associated weight loss can contribute to the appearance of facial aging due to the loss of facial volume and increased skin laxity. As fat volume in the face decreases, the cheeks, temples, and areas around the eyes may appear more hollow or sunken. Additionally, reduced skin support can lead to increased laxity, making fine lines, folds, and sagging more noticeable. Together, these changes may create the appearance of premature facial aging, particularly in individuals who experience rapid or significant weight loss.”

Swenson notes that rapid weight loss can also weaken the extracellular matrix, leading to decreased collagen synthesis and reduced elastin and fibroblast activity. This can make the skin look crepey.

What Are Some of the Latest Aesthetic Treatments for Accelerated Aging After GLP-1 Weight Loss?

Swenson recommends biostimulators, skin-tightening treatments, HA dermal fillers, and energy devices to address accelerated aging associated with rapid weight loss. Darbonne is also a fan of skin tightening treatments like Vivace Radiofrequency (RF) Microneedling and energy devices like Sofwave. 

“Treatments that stimulate collagen production and improve skin laxity can be effective in addressing facial aging associated with significant weight loss,” shares Darbonne. “Sofwave is an excellent option for moderate to more advanced skin laxity, as it lifts and tightens the skin by stimulating collagen production, elastin, and your body’s own hyaluronic acid. For patients with mild to moderate laxity, Vivace RF Microneedling is another effective treatment that helps improve skin firmness and texture while promoting collagen remodeling.”

Let’s dive a bit deeper into the benefits of these recommended treatments:

  • Energy devices (Sofwave): Sofwave stimulates collagen production in the deeper skin layers using ultrasound technology. The skin appears more lifted and tighter over time. 
  • Skin tightening treatments (SkinPen Microneedling, Vivace Radiofrequency Microneedling): Microneedling treatments create microinjuries on the skin’s surface, and radiofrequency uses heat energy that goes deeper into the skin. Both mechanisms activate collagen production to improve skin laxity. 
  • Biostimulators (Sculptra, Radiesse): Biostimulators are injected into the skin to help stimulate your own collagen production. Over time, volume is restored. 
  • HA dermal fillers (Juvederm, Restylane): HA fillers are made of hyaluronic acid and immediately restore volume when injected into the skin.

How Long Should You Wait After GLP-1 Weight Loss Before Adding HA Dermal Fillers?

Most treatments mentioned above are ideal to begin before or at the onset of GLP-1 drug use. HA fillers, on the other hand, are best used after weight loss and during the weight-loss maintenance period. 

“I would recommend adding dermal filler once the goal weight is achieved,” shares Sweonson. “Once the goal weight is achieved, most people on GLP-1 drugs use a maintenance dose to maintain the weight loss.”

Why Early Intervention Matters for Facial Rejuvenation

As is often the case, early intervention is key to mitigating or preventing signs of accelerated aging following GLP-1 weight loss. 

“The ideal time to begin these treatments is early in the weight loss journey,” shares Darbonne. “Preventing collagen loss and maintaining skin support is often easier and more effective than correcting advanced laxity after significant weight loss has already occurred. Early intervention can help preserve facial structure and maintain a more youthful appearance throughout the weight loss process.”

Using biostimulators, skin-tightening treatments, and energy devices like Sofwave at the beginning of your weight-loss journey is a great way to support your appearance and maintain a youthful, healthy look.

Bioidentical hormone replacement therapy (BHRT) can help to support GLP-1 weight loss and prevent or mitigate unwanted side effects, especially for women in the perimenopause or menopausal stages of life. 

“BHRT supports collagen,” states Swenson. “Thirty percent of collagen is lost in the first five years of menopause. If a woman is peri or menopausal, BHRT is essential to maintain skin, bones, and hair.”
